In the wake of the 2013 Interstate 5 bridge collapse in Oregon involving an oversize load, the United States Government Accountability Office (GAO) is urging the federal government to implement guidelines to improve each state’s oversize load permitting process.
According to government investigations, the driver of the oversize load crashed on the Skagit River Bridge  causing it to collapse after reporting inaccurate loaded dimensions and obtaining the oversize load permit directly from the state of Washington.
The load did meet the pilot car requirements, although the investigation report suggests two pilot cars may have helped prevent the accient.
As a result, GAO is asking the Federal Highway Administration to brush up on oversize load procedures by state and develop best practices to improve safety.
